Schedule Change Request
Requesting a schedule change that exceeds 30 days or longer requires a minimum two-week notice in writing by completion of this form. All other requests off need to be submitted through the scheduling platform for manager approval.
There is NO GUARANTEE that Life Centers of Kansas can accommodate your request, but we will do our best!
Employment Classifications
Regular Full-Time Employees. Regular full-time employees are scheduled to work at least 30 hours per workweek, equal to 130 hours a in a month, except for approved PTO. Full-time employees are eligible for company benefits such as health insurance, dental insurance, vison insurance, supplemental insurances, and PTO.
Regular Part-Time / PRN Employees. Regular part-time employees are scheduled to work 30 hours or less per workweek, equal to less than 130 hours per month. Part-time/ PRN employees are not eligible for most Company benefits.
PRN status is required to work a minimum of 16 hours per calendar month.
If your request changes your employment classification you will be made aware in writing. This change can and will impact benefit eligibility.
These policies are procedures do not supersede FMLA, in accordance with the Family and Medical Leave Act of 1993 (FMLA), Life Centers of Kansas provides up to 12 or 26 weeks of unpaid, job-protected leave in a 12-month period to covered employees in certain circumstances. See employee handbook section 7.11 for full explanation of this benefit.
